In today’s market, ensuring a sound and thorough Contractor Prequalification Process is critical to accomplish smooth and efficient project completion. • Similar contracts recently completed
• Company’s current backlog
• Employer Modification Rate (EMR) as reported by the Company’s insurance carrier
• Sample Insurance Certificate reflecting coverage as required
• OSHA 300 logs
• Financial Statements
• Dun and Bradstreet Report
• Bonding Capacity
• Licensing Information
• Business Size Information
• Additional Commercial and Technical Information as applicable to bid package
• Total Recordable Incident Rate (TRIR) based on submitted OSHA 300 logs
• Lost Workday Cases rates based on submitted OSHA 300 logs and compared to the national averages for companies with the same NAICS or SIC code
• Evaluation of the effectiveness of the contractor’s overall safety program
• Development and implementation of the Contractor’s health and safety procedures
• Verification of a corporate substance abuse program
• Employee safety training programs and records
• OSHA citations issued for the past three  years
• Reported fatalities or published by OSHA website.
